# How to Start a Homeopathy Clinic in India: A Practical Checklist

> Start with a recognised homeopathy qualification, usually BHMS, and register with your State Board or Council of Homoeopathy. Then register the clinic as your state requires: clinical establishment registration where that law applies, a shop and establishment or trade licence, bio-medical waste authorisation if needed, and GST once turnover crosses the threshold. Confirm each with local authorities.

## Key takeaways

- Practising needs a recognised qualification and registration with your State Board or Council of Homoeopathy.
- The NCH replaced the Central Council of Homoeopathy in 2021 and maintains the National Register.
- The Clinical Establishments Act, 2010 covers AYUSH clinics but applies only where a state has adopted it.
- Bio-medical waste authorisation applies to clinics handling such waste, whatever the quantity.
- GST registration for services applies above ₹20 lakh turnover (₹10 lakh in some states); ask a CA.
- Set up records, appointments and billing before the first patient arrives.

To start a homeopathy clinic in India you need a recognised homeopathy qualification (usually BHMS), registration with your State Board or Council of Homoeopathy, and the local registrations your state requires for the premises, such as clinical establishment registration where that law applies and a shop and establishment or trade licence. Add bio-medical waste authorisation if you generate such waste, and GST registration if your turnover crosses the threshold.

Rules differ from state to state, so use this as a checklist and confirm each item with your state authority before you open.

## What qualification and registration do you need to practise?

The starting point is a recognised qualification in homeopathy, usually the BHMS degree from a college recognised by the National Commission for Homoeopathy (NCH). The NCH is the statutory body under the Ministry of AYUSH that replaced the Central Council of Homoeopathy in July 2021, under the National Commission for Homoeopathy Act, 2020. It maintains the National Register of homoeopathic practitioners.

To practise, you register with the **State Board or Council of Homoeopathy** (the name varies by state) in the state where you will work. Keep these points in mind:

- Registration is state-specific. If you move states, check what the new state requires.
- Your registration number belongs on your prescriptions and letterhead; many states expect it displayed in the clinic as well.
- Renewal periods and fees differ by state. Diary the renewal date the day you register.
- Keep certified copies of your degree, internship completion and registration certificates together; the clinic registrations below usually ask for them.

If you completed an internship recently, make sure your registration is permanent rather than provisional before opening independently.

## Which registrations does the clinic itself need?

Your personal registration lets you practise. The clinic, as premises and as a business, usually needs its own registrations. The common ones are:

| Registration | Who issues it | When it applies |
| --- | --- | --- |
| Clinical establishment registration | State or district registering authority | In states that have adopted the Clinical Establishments Act, 2010, or have their own equivalent law |
| Shop and establishment or trade licence | State labour department or local municipal body | Depends on state law and your local body |
| Bio-medical waste authorisation | State Pollution Control Board or Committee | If you generate bio-medical waste, irrespective of quantity |
| GST registration | GST department | When turnover crosses the threshold, or in other cases set by law |
| Signboard or advertising permission | Local municipal body | In some cities, for outdoor signs |

**Clinical Establishments Act.** The Clinical Establishments (Registration and Regulation) Act, 2010 requires clinics to register, and it covers AYUSH establishments as well as allopathic ones. It does not apply everywhere automatically: states adopt it, or pass their own law, and several states have their own clinic registration rules. Ask your district health office which law applies to you.

**Bio-medical waste.** The Bio-Medical Waste Management Rules, 2016 cover clinics and dispensaries. Any facility handling bio-medical waste needs authorisation from the State Pollution Control Board or Pollution Control Committee, whatever the quantity. A homeopathy clinic that never uses needles or dressings may generate very little, but check your state's position rather than assuming.

## Do you need GST registration for a homeopathy clinic?

Under GST law, a business that supplies services must register once its aggregate turnover in a year crosses ₹20 lakh, or ₹10 lakh in some special category states. For businesses supplying only goods, the threshold in most states is ₹40 lakh.

For a clinic, the picture has two parts. Healthcare services by a clinic are generally exempt from GST, while sales of medicines and other goods can be treated differently. Whether you must register, and whether any of your charges attract GST, depends on what you supply and how much. Ask a chartered accountant to review your fee structure before you open, and again as your practice grows.

## How do you choose and set up the premises?

Location matters more than décor. Look for:

- **Access and visibility.** Ground floor or a lift, near a main road or market, with somewhere to park a two-wheeler.
- **Privacy.** A consulting room where the patient cannot be overheard. Homeopathic case-taking covers intimate and emotional detail, and patients only share it when they feel safe.
- **A waiting area** that seats your busiest hour, with the reception desk in view of the door.
- **Dispensary space.** A cool, dry corner or small room, away from direct sunlight and strong smells such as perfume, camphor and cleaning chemicals, which homeopaths traditionally keep away from medicines.

Budget for the main cost heads: rent and deposit, interiors and furniture, the dispensary's initial stock and containers, a computer or tablet and printer, signage, software, and enough working capital to cover several months before the practice pays its own way. Write the numbers down; a clinic that opens with no cushion is under pressure to see patients too quickly.

## What do you need for the dispensary?

Most homeopathy clinics in India dispense their own medicines. Plan for:

- **Initial stock** of the remedies and potencies you use most, bought from licensed manufacturers or distributors. Start with what you actually prescribe and add as your practice shows you what it needs.
- **Vehicles and containers:** globules, tablets, dilution bottles, vials, droppers and labels.
- **A dispensing routine:** clear labels with the patient's name, dose and instructions, and a record of what was given.

If you plan to sell homeopathic medicines to the public, as a pharmacy, rather than dispense to your own patients, you will need a drug licence from your State Drugs Control authority. Confirm the position for your set-up before you start.

## How should you set up records, appointments and billing?

Set up your systems on day one, before the first patient, because changing them later is harder. You need a case record that captures the full homeopathic case, an appointment system that handles long first visits and short follow-ups, a way to issue prescriptions on your letterhead, and billing with receipts.

Starting directly in software saves a paper-to-digital move later. ## How do you get your first patients ethically?

A new clinic grows mostly by word of mouth from patients who were listened to and followed up. Practical steps:

- Tell your network: family doctor colleagues, local chemists, your college contacts.
- Create a Google Business Profile with your correct address, hours and registration details.
- Give every patient a clear follow-up date, and call those who miss it.
- Write or speak about homeopathy in general, educational terms.

Follow the advertising and professional conduct rules of the NCH and your State Board. Never claim to cure a named disease, and never publish patient testimonials that promise results.

## Frequently asked questions

### Can a BHMS graduate open a clinic in India?

Yes. A BHMS graduate with permanent registration from the State Board or Council of Homoeopathy can open a homeopathy clinic in that state. The clinic may also need its own registrations, such as clinical establishment registration where the state has adopted that law and a shop and establishment or trade licence. Confirm the full list with your state authorities.

### Is the Clinical Establishments Act applicable to homeopathy clinics?

The Clinical Establishments (Registration and Regulation) Act, 2010 covers AYUSH establishments, including homeopathy clinics, but it applies only in states and union territories that have adopted it. Other states have their own clinic registration laws. Ask your district health office which law applies where you plan to open.

### Does a homeopathy clinic need GST registration?

A business supplying services must register once aggregate turnover crosses ₹20 lakh a year, or ₹10 lakh in some special category states. Healthcare services by a clinic are generally exempt, but medicine sales can be treated differently. Because the answer depends on what you supply and how much, ask a chartered accountant before you open.

### Do homeopaths need a drug licence to dispense?

Most homeopathy clinics dispense medicines to their own patients as part of treatment. Running a pharmacy that sells homeopathic medicines to the public is different and needs a licence from the State Drugs Control authority. Rules and their interpretation vary, so confirm your position with the State Drugs Controller before you set up.

### Can I practise homeopathy in another state with my registration?

Registration with a State Board or Council of Homoeopathy is state-specific. If you plan to practise in another state, check with that state's Board or Council what it requires, and keep your National Register details up to date with the National Commission for Homoeopathy.
